Reception desk temporarily closed on Fridays and November, 14
Due to exceptional circumstances, the SHDM will temporarily close the reception desk every Friday starting November 11, 2022. Also note that it will be closed on Monday, November 14.
However, documents and payments can be deposited in the mailbox on the 22nd floor of 800 De Maisonneuve Boulevard East, but no receipts will be delivered.
The SHDM teams remain available every day by phone and email: 514 380-7436 and email@example.com
Thank you for your understanding and we will inform you of the resumption of our usual reception hours.
Form for registering on the list of SHDM suppliers
In accordance with the Act respecting access to documents held by public bodies and the protection of personal information, to which the SHDM is subject as a public body, the supplier consents to the disclosure of his data entered in this registration form.
The SHDM appreciates your interest and thanks you for having filled out this form. By being on our list of suppliers, we will be able, if necessary, to invite you to send us a quotation (bid).
SHDM is under no obligation of any kind to suppliers who fill out the form.
